Do not use hyperbaric oxygen treatments if you have symptoms or conditions identified in this contraindications list.

Before investing in a hyperbaric oxygen chamber, we recommend seeking advice from a medical professional.

I acknowledge that I do waive and release Hyperbaric Oxygen Therapy Christchurch including all personnel, owners, and management, of any, and all liability for loss, damage, expense, or injury that I may suffer while undertaking mild Hyperbaric Oxygen therapy (HBOT) treatment in their clinic and/or during my use of any HBOT devices that I may now or in the future rent or purchase from them.

I acknowledge I have been informed of all potential harm and injury that may possibly result from my usage of mHBOT, for which mHBOT may have contraindications.

I understand that these contraindications may result in unfavourable outcomes that may further complicate my condition or even in the worst case, contribute to death when using mHBOT.

I further acknowledge that I have been fully briefed and advised by staff of Hyperbaric Oxygen Therapy Christchurch to seek medical practitioner advice in regard to my intention to undergo mHBOT.
